Position Summary

Overview:
Coordinate transplant activities: completing pre-transplant evaluations, preparation and presentation to the medical review board, coordination of organ transplant process, inpatient care including discharge teaching, planning for outpatient care and post-operative follow-up.

Responsibilities:
Transplant referral and evaluation: facilitate/coordinate evaluation and OPTN/UNOS listing of potential transplant candidate. Pre-transplant waiting period: ability to monitor and coordinate care of the candidate awaiting organ transplantation. Peri-operative period: ability to facilitate transplant process when organ becomes available to transplant candidate. Post transplant inpatient period: understanding of post-transplant management including medications, complications, interventions and discharge planning; the ability to collaborate with multidisciplinary team. Post transplant outpatient period: identify and respond to recipient/family needs in post-transplant phase; reinforce education and follow-up requirements necessary for successful outcome; observe, monitor, assess and report the recipient's physical and emotional status, short and long-term complications of immunosuppression, drug toxicity, infectious complications; assist family with recipient¿s physical, social and emotional rehabilitation; assess compliance with medications and medical regimen; reinforce post-operative teaching; collect and review health and organ function data; communicate lab data and diagnostic results; implement changes in drug/treatment therapy as ordered; arrange for consultations, diagnostic procedures and hospitalization; reinforce and facilitate need for well care; ensure recipient is screened annually for malignancy; assess recipient's need for immunizations; maintain communication with referring physician, other heath care providers and payors; provide and maintain comprehensive documentation of recipient's progress. Living donation: ability to facilitate living donation/transplantation and coordinate the care of the living donor for evaluation and live organ donation. Professional development: obtain ABTC certification; maintain ABTC certification.

Knowledge and Skills:
Working knowledge of renal, liver and lung pathophysiology and the transplant process required. The ability to demonstrate and perform generic and unit-based/critical competencies including the specific care of age and diagnosis appropriate patients; demonstrate positive interpersonal skills; utilize time and resources in a cost effective manner; demonstrate flexibility; demonstrate excellent verbal and written communication skills; delegation; operate basic medical equipment; read and analyze basic medical equipment; utilize a personal computer to include word processing and basic office equipment. The ability to show diplomacy and sensitivity in caring for families facing end-stage organ failure of their child, ability to coordinate an interdisciplinary health care team efficiently and in a collaborative manner.

Education, Licensure and Experience:
Current Texas RN license and a minimum three (3) years pediatrics or solid organ transplant field experience required. Bachelor's degree in nursing and experience in solid organ transplant or related field preferred.
